How to Delete FaceBook Account Permanently

Question : FaceBook is good for Business & users in order to connect with others but the only problem which i have with this service is that, its actually bringing in many kids and children waste their time. My nephew is wasting most of his time on playing games on FaceBook like Farmville, Poker etc and creating multiple accounts just to send gifts to different accounts everyday. Hence, he has also created an account using my office email address and i want to get it closed, can you suggest me on how to delete the FaceBook account completely so that my privacy is secure and no one can find that my FaceBook Profile was every existed.
